Privacy notice

What the contact form actually collects, how a clinical record would be kept and shared, and the analytics or advertising trackers this site does not run.

This is a demonstration website. Woodside Vet & Care, LLC is a fictional practice, but the code behind this site and the hosting it runs on are both real, so everything below is a genuinely accurate description of what this particular website does with anything you type into it.

What the contact form collects

The only place on this site that asks you for anything is the form on the contact page. It asks for:

  • Your name
  • Your email address, so we can reply
  • An optional telephone number, if you would rather we called
  • Your animal's name, species or breed, and its rough age, if you give it to us
  • Roughly when suits you to bring your animal in
  • What your message is about, chosen from a short list of topics
  • Whatever you write in the message field itself

Nothing else on the site asks you for anything, and no hidden field collects more than what is printed on the form in front of you.

What happens after you press send

This site has no database, admin panel or spreadsheet that your message lands in. Submitting the form sends your fields to an endpoint run by MaiaHost, the hosting platform this demonstration uses. That endpoint checks a single-use token issued when the page loaded and a captcha, both there to filter out automated and repeated submissions, and only once those checks pass does it deliver your message as an email to a mailbox on this domain.

Because Woodside Vet & Care, LLC is not a real practice, nobody reads that mailbox as a veterinarian, and nothing you send results in an appointment or advice about an actual animal — it exists only to demonstrate that the form works.

Cookies, analytics and third-party trackers

This site sets no analytics cookies and no advertising cookies, and it does not embed a third-party analytics platform, an advertising pixel, a social-media widget or a chat bot on any page. There is no cookie banner because there is genuinely nothing here to ask your consent for.

The one piece of code loaded from outside this domain is the contact-form script referred to above, used only on the contact page to submit your message and run the checks described above — not to follow you around the web or build an advertising profile.

No visitor profile is built

There is no login, no account, no persistent identifier set to recognise you on a return visit, and no cross-site tracking of any kind. Submitting the form tells us what is in the form; browsing the rest of the site does not add anything to a file about you, because no such file exists.

Clinical records: what we would keep, and for how long

If this were a real practice, every animal we saw would have a clinical record: the history you give us, what we find on examination, test results, images, and what we diagnose and do about it. That record belongs to the animal's ongoing care rather than to any single visit, and would not be deleted simply because you had not been in for a while.

Our policy would be to keep a clinical record for your animal's working life and for a minimum of seven years after its last visit, whichever is longer, because an old record can matter to a future veterinarian even once it has stopped mattering to us. We would keep it for that same period if your animal passed away or you told us you had rehomed it, for the same reason.

Who a clinical record would be shared with

A record would only go to three kinds of recipient, and only for a reason connected with your animal's own care:

  • A referral practice or specialist

If we referred your animal for treatment we do not provide, we would send enough of the record for the next practice to pick the case up without repeating tests already done.

  • A laboratory

Where a sample needed a test our in-house laboratory could not run, we would send the sample and its request, not the whole record.

  • Your insurer

Only when you made a claim, and only the information relevant to that claim, sent because you asked us to rather than as routine.

We would not sell, rent or otherwise share a record with anyone else, including for marketing, or hand one to another owner, a landlord or an employer without your say-so.

Your rights over your own data

You could ask to see what is held about you and your animal, ask us to correct anything wrong, and ask us to stop using it for anything beyond your animal's direct care. One honest limit: a completed clinical record could not simply be deleted on request, because keeping it for the period above protects your animal as much as it protects us. Anything that is not a clinical record, such as your contact details, would not carry that limit.

Getting a copy, or moving to another practice

Ask, in whatever way suits you, and we would send your complete records anywhere you like, free, the same working day. We would not ask why you were moving, or make transferring difficult in the hope you stayed. There is no self-service account to log into, because there would be no account system at all — a plain conversation with the desk is the whole mechanism.

Ordinary web server logs

Like effectively every website, the server this site runs on keeps a short-term access log — the requesting IP address, the time, the page requested and the browser's user agent string — purely for security and troubleshooting. That log is not cross-referenced with anything submitted through the contact form, is not used to build a profile of anyone, and is not shared with or sold to anyone.

What this notice does not claim

This is a plain, specific description of what this small demonstration website actually does, not a claim of compliance with any privacy law, and not a certification of any kind. A real veterinary practice handling real clinical and financial records would need far more than this page; this one needs exactly this much, because collecting a name, an email address and a message, and emailing it on, is the whole of what this site does.

Because this is a demonstration

No real client or patient data exists. Woodside Vet & Care, LLC is a fictional practice, so nothing described above under "clinical records" or "who a record would be shared with" has ever actually happened — it is written the way a genuine practice's notice would read, not because a single record has ever been created. The one genuinely real part of this page is the contact form: a managed form provided by MaiaHost, delivering any message sent through it to a mailbox kept for this demonstration, not to a veterinary practice, and never resulting in an appointment or advice about a real animal.
